Yes he’s an awesome therapist and I’ve had him for 3 years. He told me he’d rather me cut myself instead of smoking crack because cutting myself or exercising self injurious behavior was the lesser of two evils and that if he had do do damage control that cutting myself would be ok.

Members responded with empathy and concern to a question about whether a therapist might approve of self-harm as 'harm reduction' compared to drug use. Several members shared that they had never heard a therapist recommend this approach, with some expressing outrage while others acknowledged understanding the concept even if they did not support it. A recurring theme was the community rallying around the original poster with love, prayers, and encouragement, especially celebrating her 11 months of sobriety and reminding her she is not alone.

Oh, Nikki, I am so very sorry that "harm reduction" even has to be a thing for you. I don't condone self-harm in any way, shape or form, or illicit drug use and it absolutely sucks moose meat that you have to distract your mind from your emotional pain by causing yourself physical pain. I'm not going to tell you to stop, but I damned sure I'm going to wish and hope you can.

We are here for you, Dear One, however we can be.

That's a really important question, and it sounds like there's a lot of trust built between you and your therapist over those 3 years, which is genuinely valuable.

What your therapist may be describing is a concept called "harm reduction." The idea is that when someone is facing two harmful choices, guiding them toward the less dangerous one can be a practical short-term strategy while working toward healthier coping skills overall.

That said, self-injury is never truly "safe," and most clinical guidelines would not recommend it as an endorsed coping strategy. Risks can include:

- Infection or accidental serious injury
- The behavior becoming harder to stop over time
- Emotional shame or guilt that can worsen depression

It's worth having an open, honest conversation with your therapist about what the longer-term plan looks like — because "damage control" should really be a bridge, not a destination.

If you ever feel unsafe or that the self-injury is escalating, please reach out to the 988 Suicide & Crisis Lifeline by calling or texting 988. You deserve support that moves toward healing, not just managing harm.
